Rental - Income Calculation Form (AHP VHG R MON 117)

For AHP homeownership projects awarded prior to 2019, this form must be completed for each household receiving Affordable Housing Program funds to accurately verify household income. For AHP and voluntary housing grant rental projects, this form must be completed during tenant intake and will accompany income documentation provided during monitoring.
